Known issue: Samsung top-load washer top can detach in spin (2016 CPSC recall)

In November 2016 Samsung and the CPSC recalled about 2.8 million top-load washers (34 models, sold March 2011 to November 2016, mid-control and rear-control styles) because the top could separate from the chassis during spin. Inclusion depends on model AND serial, so the only reliable check is Samsung's recall lookup. Units that got the remedy have a reinforced top; units that were never fixed still show up on service calls.
